BRAMS is currently recruiting for full-time Social Workers based in Broome, WA, across both aged care and disability (NDIS) programs.
Based on your experience, interests, and strengths, you will be supported to work within the team where you can make the greatest impact — whether that’s supporting older people through life transitions, or working alongside individuals with disability to build independence and community connection.
In this role, you will be part of a multidisciplinary team delivering holistic, person-centred care that supports individuals, families, and community across all stages of life.
This is a relationship-driven role, where success comes from building trust, listening deeply, and working alongside people to ensure individuals are empowered to make their own choices in a way that respects their culture, values, and lived experience.
More specifically, your responsibilities include but are not limited to:
- Delivering person-centred social work services across aged care and/or NDIS programs
- Conducting psychosocial assessments to understand social, emotional, and functional needs
- Developing care plans and support plans in partnership with individuals, families, and carers
- Providing counselling and emotional support around grief, loss, mental health, ageing, and disability
- Supporting individuals to build capacity, independence, and participation in community life
- Facilitating group programs and individual sessions that strengthen wellbeing and connection
- Supporting individuals and families through important life transitions with care and sensitivity
- Collaborating with multidisciplinary teams and community partners to deliver integrated support
- Maintaining accurate documentation and case notes in line with program requirements

Is This You?
To succeed in this role, you will hold a Degree in Social Work (or equivalent) and hold (or be eligible for) membership with the Australian Association of Social Workers (AASW). You will also have demonstrated experience working in aged care, disability, mental health, or community services, with a strong foundation in person-centred and culturally safe practice.
Additionally, the following skills and experience will be highly valued:
- Experience providing counselling and psychosocial support across complex life situations
- Understanding of the aged care system and/or NDIS frameworks
- Experience supporting individuals and families through grief, loss, disability, or life transitions
- Ability to develop and implement person-centred care and support plans
- Experience working collaboratively within multidisciplinary teams
- Experience in remote, regional, or Aboriginal community settings (highly regarded)
